At JDRF, protecting the health and safety of our JDRF guests, donors, volunteers and staff remains a top priority as we prepare to come together in person to advance the mission we all share. For this reason, we are requiring all attendees and volunteers to (1) a negative COVID test administered within 72 hours of attendance to the Gala or provide proof of vaccination , and (2) acknowledge and agree to the terms set forth in the COVID-19 Assumptions of Risk and Waiver of Liability Agreement (“the Agreement”), including but not limited to the waiver of liability.
We look forward to providing a positive, low risk experience for all event attendees. To help with that, please come prepared to show one of the following during the event check-in process:
A printed or digital negative COVID-19 PCR or antigen test (rapid test) administered by a healthcare professional with 72 hours to a guest’s given event that includes the guest’s name and date the last test was administered
An official, government-issued vaccination card listing the guest’s name and dates the last dose was administered
A photo or digital version of an official government-issued vaccination card listing the guest’s name and dates the last dose was administered
WARNING: Although JDRF has implemented health and safety measures in accordance with applicable federal, state, and local requirements, no precautions can completely eliminate the risks of exposure to COVID-19. Attendance at the Gala presents an inherent risk of possible exposure, infection, and/or transmission of COVID-19. By attending the Gala, all guests assume the risk of infection and/or transmission of COVID-19.
WAIVER OF LIABILTY: By attending the Gala, I, on my own behalf and on behalf of my heirs, executors, personal representatives, administrators, and assigns, agree to waive, and forever discharge JDRF, and its directors, officers, employees, agents, clients, customers, vendors, and any other representative of the Company (the “Released Parties”), to the maximum extent permitted by law, from any and all claims, liability, and damages of whatever kind or nature, including but not limited to those: (1) caused by the ordinary negligence, fault, or other action or omission of the Released Parties; and (2) arising in connection with the Gala.
